{"id":161502,"date":"2020-06-05T07:30:58","date_gmt":"2020-06-05T07:30:58","guid":{"rendered":"https:\/\/dentaltraumaguide.org\/history\/"},"modified":"2020-08-04T08:23:16","modified_gmt":"2020-08-04T08:23:16","slug":"history","status":"publish","type":"page","link":"https:\/\/dentaltraumaguide.org\/pt\/history\/","title":{"rendered":"Hist\u00f3ria"},"content":{"rendered":"<h2>Hist\u00f3ria do <em>Dental Trauma Guide<\/em><\/h2>\n<p>Em 1962 foi tomada uma importante decis\u00e3o no Centro de Traumas do Departamento de Cirurgia BucoMaxilofacial do Hospital Universit\u00e1rio de Copenhague: seria feito um acompanhamento peri\u00f3dico de todos os pacientes tratados por les\u00f5es traum\u00e1ticas dent\u00e1rias e maxilofaciais. Na \u00e9poca, o departamento era o \u00fanico respons\u00e1vel pelo tratamento das les\u00f5es traum\u00e1ticas dent\u00e1rias na parte oriental da Dinamarca, com uma popula\u00e7\u00e3o de cerca de dois milh\u00f5es de habitantes.<\/p>\n<p>Os princ\u00edpios de tratamento seguidos na \u00e9poca foram baseados no livro de 1960\u00a0<em>\u201dThe Classification and Treatment of Traumatic Dental Injuries\u201d<\/em>, de R.G. Ellis (1), um livro que se baseava em relatos de casos e opini\u00f5es de especialistas. Devido \u00e0 falta de bom material cient\u00edfico no campo da traumatologia dent\u00e1ria, tal baixo n\u00edvel de evid\u00eancia forneceu a \u00fanica base para a tomada de decis\u00f5es de tratamento.<\/p>\n<p>Logo se tornou evidente nas visitas de acompanhamento de pacientes traumatizados que os tratamentos propostos com base nessas fontes de informa\u00e7\u00e3o frequentemente levavam a s\u00e9rias complica\u00e7\u00f5es. Estas complica\u00e7\u00f5es foram evidenciadas em uma s\u00e9rie de estudos retrospectivos, abrangendo\u00a0les\u00f5es de luxa\u00e7\u00e3o\u00a0(7),\u00a0avuls\u00f5es\u00a0seguidas de\u00a0reimplante\u00a0(2, 3),\u00a0fraturas radiculares\u00a0(4, 5),\u00a0fraturas alveolares\u00a0(6) e\u00a0fraturas de mand\u00edbula\u00a0(6). As complica\u00e7\u00f5es de cicatriza\u00e7\u00e3o identificadas nestes acompanhamentos cl\u00ednicos e radiogr\u00e1ficos foram\u00a0necrose pulpar,\u00a0oblitera\u00e7\u00e3o da cavidade pulpar,\u00a0reabsor\u00e7\u00e3o radicular progressiva,\u00a0perda do ossos de sustenta\u00e7ao\u00a0e\u00a0perda do dente.<\/p>\n<p>Uma tentativa de estabelecer uma rela\u00e7\u00e3o causa-efeito entre tipo e extens\u00e3o do trauma, modalidade de tratamento e posterior desenvolvimento de complica\u00e7\u00f5es foi seriamente prejudicada pelos dados incompletos presentes nos prontu\u00e1rios dos pacientes, bem como pela documenta\u00e7\u00e3o radiogr\u00e1fica inicial insuficiente sobre a extens\u00e3o da les\u00e3o. O primeiro passo para corrigir essa situa\u00e7\u00e3o foi estabelecer um sistema de classifica\u00e7\u00e3o das les\u00f5es de traumatismo dent\u00e1rio abrangendo todas as poss\u00edveis les\u00f5es dos tecidos dent\u00e1rios mineralizados, dos tecidos periodontais e do osso alveolar. Como modelo, foi selecionado o sistema de classifica\u00e7\u00e3o da Organiza\u00e7\u00e3o Mundial da Sa\u00fade (OMS) e acrescentadas alguns tipos de trauma ausentes (concuss\u00e3o, subluxa\u00e7\u00e3o e luxa\u00e7\u00e3o lateral) para incluir todos os cen\u00e1rios de les\u00e3o biologicamente distingu\u00edveis (147). Este sistema de classifica\u00e7\u00e3o ganhou desde ent\u00e3o uma ampla aceita\u00e7\u00e3o e tem sido utilizado em todos os nossos estudos subsequentes sobre traumatismos.<\/p>\n<p>Para melhorar a qualidade dos registros de trauma dos pacientes no Hospital Universit\u00e1rio e aumentar seu valor para a pesquisa, foi desenvolvido e implementado em 1966 um formul\u00e1rio de trauma padronizado. Esse prontu\u00e1rio padronizado era bastante abrangente e continha todos os detalhes necess\u00e1rios sobre o tipo e a extens\u00e3o da les\u00e3o, caracter\u00edsticas do paciente (por exemplo, est\u00e1gio de rizog\u00eanese, idade e sexo), assim como informa\u00e7\u00f5es sobre o tratamento fornecido. Ao todo foram coletadas 65 informa\u00e7\u00f5es sobre o trauma para cada paciente. Em 1969 um exame radiogr\u00e1fico padronizado foi desenvolvido utilizando 4 radiografias de cada dente traumatizado e em 1971 uma documenta\u00e7\u00e3o fotogr\u00e1fica foi adicionada para incluir uma foto facial da regi\u00e3o bucal para registrar les\u00f5es labiais associadas, assim como uma foto frontal e uma foto axial dos dentes lesionados (16). Para complementar os registros iniciais, foram realizados procedimentos de acompanhamento em intervalos pr\u00e9-determinados, escolhidos com base na experi\u00eancia e representando os tempos ideais para a identifica\u00e7\u00e3o de complica\u00e7\u00f5es. Sempre que o tratamento falhava e os dentes eram removidos, eles eram examinados no Departamento de Patologia Oral da Faculdade de Odontologia que tinha fortes liga\u00e7\u00f5es com o Hospital Universit\u00e1rio (o autor s\u00eanior tendo trabalhando em ambas as institui\u00e7\u00f5es). Nas d\u00e9cadas de 70 e 80 foram acumulados dados padronizados suficientes para permitir\u00a0estudos cl\u00ednicos prospectivos sobre traumatismos e foram conclu\u00eddas um total de 80 publica\u00e7\u00f5es abrangendo todas os tipos de trauma que afetam tanto a denti\u00e7\u00e3o dec\u00eddua quanto a denti\u00e7\u00e3o permanente (2-80, 156).<\/p>\n<p>No in\u00edcio dos anos 70, uma oportunidade \u00fanica de fazer\u00a0traumatologia dent\u00e1ria experimental\u00a0ocorreu quando o autor s\u00eanior foi autorizado a conduzir pesquisas com macacos usados no Instituto do Soro da Dinamarca (<em>Danish Serum Institute)<\/em>; estes animais eram usados como doadores de rins para a produ\u00e7\u00e3o de vacina contra a poliomielite. Os experimentos realizados com esses macacos resultaram em 65 estudos experimentais em traumatismo dent\u00e1rio, incluindo alguns estudos <em>in vitro<\/em> (81-146).<\/p>\n<p>Na se\u00e7\u00e3o seguinte ser\u00e1 descrito o volume de informa\u00e7\u00f5es cl\u00ednicas e experimentais dispon\u00edveis para cada tipo de trauma. Esta informa\u00e7\u00e3o \u00e9 essencial pois molda a abordagem utilizada pelo\u00a0<em>Dental Trauma Guide<\/em>\u00a0para cada les\u00e3o traum\u00e1tica individualmente em rela\u00e7\u00e3o ao\u00a0diagn\u00f3stico, escolha do tratamento\u00a0e\u00a0estimativa de progn\u00f3stico.<\/p>\n<h3>Les\u00f5es por luxa\u00e7\u00e3o<\/h3>\n<p>Concuss\u00e3o, subluxa\u00e7\u00e3o, luxa\u00e7\u00f5es extrusivas, laterais e intrusivas envolvendo a denti\u00e7\u00e3o permanente foram analisadas em uma s\u00e9rie de estudos prospectivos (16, 17, 20, 21, 22, 66-69) e complica\u00e7\u00f5es como necrose pulpar, oblitera\u00e7\u00e3o da cavidade pulpar (22), reabsor\u00e7\u00e3o radicular externa e interna (29), perda \u00f3ssea marginal transit\u00f3ria e permanente (16, 17) e reabsor\u00e7\u00e3o \u00f3ssea apical transit\u00f3ria foram analisadas em detalhes (21-46). A etiologia e patog\u00eanese da\u00a0necrose pulpar\u00a0foi examinada especialmente em rela\u00e7\u00e3o ao tipo de luxa\u00e7\u00e3o e est\u00e1gio de rizog\u00eanese (17). A etiologia e patologia da\u00a0oblitera\u00e7\u00e3o da cavidade pulpar\u00a0(OCP) foi outra modalidade de cicatriza\u00e7\u00e3o pulpar estudada e foi encontrada uma forte rela\u00e7\u00e3o entre a ocorr\u00eancia de OCP e o deslocamento inicial (corte do suprimento apical de sangue) e o est\u00e1gio de rizog\u00eanese (22). O efeito a longo prazo da OCP (ou seja, at\u00e9 20 anos) foi analisado em um estudo conjunto com o Dr. A. Robertson da Faculdade de Odontologia de Gotemburgo, Su\u00e9cia. Observou-se um pequeno aumento anual no risco de necrose pulpar subsequente \u00e0 OCP (37).<br \/>\nForam analisadas as complica\u00e7\u00f5es da cicatriza\u00e7\u00e3o periodontal tendo sido constatada uma forte rela\u00e7\u00e3o entre os eventos de reabsor\u00e7\u00e3o radicular e o tipo de luxa\u00e7\u00e3o (66-69). Al\u00e9m disso, para intrus\u00f5es, foi encontrada uma rela\u00e7\u00e3o entre a escolha do tratamento inicial e a frequ\u00eancia da reabsor\u00e7\u00e3o radicular posterior. (51-53).<\/p>\n<h3>Les\u00f5es por avuls\u00e3o seguidas de reimplante<\/h3>\n<p>O primeiro estudo cl\u00ednico\u00a0retrospectivo\u00a0de 110 dentes replantados foi publicado em 1966 e mostrou uma rela\u00e7\u00e3o significativa entre a dura\u00e7\u00e3o do per\u00edodo \u00a0extra-oral a seco e as complica\u00e7\u00f5es de cicatriza\u00e7\u00e3o futuras, como necrose pulpar e reabsor\u00e7\u00e3o radicular progressiva (2, 3). Um estudo de 22 dentes humanos extra\u00eddos e histologicamente examinados mostrou que os eventos de reabsor\u00e7\u00e3o radicular poderiam ser divididos em reabsor\u00e7\u00e3o superficial, reabsor\u00e7\u00e3o inflamat\u00f3ria e reabsor\u00e7\u00e3o por substitui\u00e7\u00e3o (2, 3); uma terminologia posteriormente alterada para\u00a0<em>reabsor\u00e7\u00e3o relacionada ao reparo,\u00a0reabsor\u00e7\u00e3o inflamat\u00f3ria relacionada \u00e0 infec\u00e7\u00e3o\u00a0e\u00a0reabsor\u00e7\u00e3o relacionada \u00e0 anquilose (substitui\u00e7\u00e3o \u00f3ssea).<\/em><\/p>\n<p>Um estudo histol\u00f3gico demonstrou que a reabsor\u00e7\u00e3o relacionada \u00e0 infec\u00e7\u00e3o teve sua origem na presen\u00e7a de bact\u00e9rias localizadas no canal radicular e t\u00fabulos dentin\u00e1rios (3) e um estudo cl\u00ednico mostrou que o tratamento convencional do canal radicular poderia prevenir e deter a reabsor\u00e7\u00e3o relacionada \u00e0 infec\u00e7\u00e3o em cerca de 10% dos casos (2). Alguns anos depois foi descoberto que o hidr\u00f3xido de c\u00e1lcio foi eficaz na preven\u00e7\u00e3o e parada da reabsor\u00e7\u00e3o relacionada \u00e0 infec\u00e7\u00e3o em 90% dos casos (12).<\/p>\n<p>No in\u00edcio dos anos 70, uma s\u00e9rie de experimentos em macacos foi iniciada para estudar a patog\u00eanese dos tipos de reabsor\u00e7\u00e3o radicular posterior ao reimplante (88-99), e para avaliar o efeito de v\u00e1rios procedimentos de tratamento sobre o resultado do reimplante dos dentes permanentes. Entre os procedimentos e fatores testados estavam o efeito da remo\u00e7\u00e3o do\u00a0<em>co\u00e1gulo<\/em>\u00a0antes do reimplante (93), o efeito prejudicial causado pelo\u00a0<em>tempo extra oral a seco e \u00famido<\/em>, a efic\u00e1cia de v\u00e1rios\u00a0<em>meios de armazenamento\u00a0e suas\u00a0temperaturas<\/em>\u00a0(95, 109, 116, 141),\u00a0<em>procedimentos de limpeza<\/em>\u00a0da superf\u00edcie radicular antes do reimplante (104),\u00a0<em>exatid\u00e3o do reposicionamento<\/em>\u00a0(102, 113),\u00a0<em>m\u00e9todos de conten\u00e7\u00e3o<\/em>\u00a0(r\u00edgida ou flex\u00edvel) e\u00a0<em>dura\u00e7\u00e3o do tempo<\/em> <em>de conten\u00e7\u00e3o<\/em>\u00a0(81, 110<em>),\u00a0sobrecarga oclusal\u00a0durante a cicatriza\u00e7\u00e3o<\/em> (101), e\u00a0<em>tratamento endod\u00f4ntic<\/em>o\u00a0antes do reimplante (103, 106, 120, 121, 125-129). Naquela \u00e9poca, foi organizada uma colabora\u00e7\u00e3o com colegas da Faculdade de Odontologia de Estocolmo (Drs. L. Hammarstr\u00f6m, L. Andersson, S. Lindskog, L. Bl\u00f6mlof) e Dr. M. Cvek do Instituto Odontol\u00f3gico <em>Eastman<\/em> em \u00a0Estocolmo (117, 118) voltada para o desenho das pesquisas experimentais quanto aos procedimentos experimentais e classifica\u00e7\u00e3o histol\u00f3gica dos tipos de cicatriza\u00e7\u00e3o. Da mesma forma, uma s\u00e9rie de experimentos sobre os efeitos de\u00a0<em>antibi\u00f3ticos <\/em>na cicatriza\u00e7ao pulpar e periodontal ap\u00f3s o reimplante tamb\u00e9m foi realizada por esses autores (143-146).<\/p>\n<p>Em 1994 foi publicado o primeiro estudo\u00a0prospectivo\u00a0de 400 dentes humanos \u00a0com per\u00edodos de acompanhamento de at\u00e9 20 anos ap\u00f3s o reinplante. Devido \u00e0s informa\u00e7\u00f5es detalhadas sobre a rela\u00e7\u00e3o significativa entre a condi\u00e7\u00e3o de armazenamento extra-alveolar e a ocorr\u00eancia de complica\u00e7\u00f5es pulpares e periodontais posteriores, foi poss\u00edvel estabelecer uma detalhada an\u00e1lise de sobreviv\u00eancia para dentes reimplantados com um determinado conjunto de condi\u00e7\u00f5es extra-alveolares (33-36). Em um estudo posterior foi descrita a influ\u00eancia de diferentes fatores tais como idade, conten\u00e7\u00e3o e t\u00e9cnicas endod\u00f4nticas na progress\u00e3o ou paralisa\u00e7\u00e3o de reabsor\u00e7\u00f5es radiculares (64, 65).<\/p>\n<h3>Fraturas coron\u00e1rias com e sem exposi\u00e7\u00e3o da polpa<\/h3>\n<p>Atualmente, estamos trabalhando em um estudo prospectivo que abrange 853 dentes permanentes, incluindo os cinco tipos de fraturas de coroa (infra\u00e7\u00f5es, esmalte, esmalte-dentina e esmalte-dentina-polpa), bem como combina\u00e7\u00f5es com os 5 tipos de luxa\u00e7\u00e3o acima mencionados (76-80). Para todos os tipos de fraturas e suas combina\u00e7\u00f5es, taxas de cicatriza\u00e7\u00e3o pulpar e periodontal s\u00e3o documentadas.<\/p>\n<p>Em 1985 um novo sistema adesivo (Gluma\u00ae) foi desenvolvido pelo Dr. Asmussen e pelo Dr. Munksgaard na Escola de Odontologia em Copenhague em coopera\u00e7\u00e3o com a empresa ESPE\u00ae e foi decidido usar o sistema para a colagem de fragmentos coron\u00e1rios recuperados (27, 28). Um estudo multic\u00eantrico escandinavo, publicado em 1991 (Gothenburg, Su\u00e9cia, Oslo, Noruega e Copenhage, Dinamarca) demonstrou que o m\u00e9todo teve alguns resultados promissores no longo prazo (31), mas fragmentos colados se soltaram frequentemente devido \u00e0 mastiga\u00e7\u00e3o ou a um novo trauma, ambos os eventos indicando a necessidade de melhorar ainda mais o procedimento de colagem, um fato que indicou uma s\u00e9rie de novos estudos\u00a0<em>in vitro<\/em>\u00a0(132-137).<\/p>\n<h3>Fraturas coronorradiculares<\/h3>\n<p>As duas estrat\u00e9gias de tratamento mais comuns para fraturas coronorradiculares n\u00e3o complicadas e complicadas s\u00e3o: \u00a0gengivectomi, com ou sem osteotomia, seguida (s) de restaura\u00e7\u00e3o e extrus\u00e3o ortod\u00f4ntica e gengivectomia seguidas de restaura\u00e7\u00e3o. Estas duas alternativas foram analisadas em um estudo prospectivo de longo prazo. Tendo sido encontradas diferen\u00e7as significativas entre os dois m\u00e9todos, a favor da extrus\u00e3o ortod\u00f4ntica, no que se refere \u00e0 cicatriza\u00e7\u00e3o a longo prazo quando consideradas a estabilidade do dente restaurado na arcada dent\u00e1ria e a sa\u00fade gengival ao dente restaurado.<\/p>\n<h3>Fraturas radiculares<\/h3>\n<p>Em 1967, o primeiro estudo cl\u00ednico, radiogr\u00e1fico e histol\u00f3gico\u00a0<em>retrospectivo<\/em>\u00a0de 50 dentes permanentes anteriores portadores de fraturas radiculares revelou tr\u00eas tipos principais de progn\u00f3stico: cicatriza\u00e7\u00e3o com\u00a0<em>tecido mineralizado entre os fragmentos<\/em>, ,\u00a0<em>cicatriza\u00e7\u00e3o com interposi\u00e7\u00e3o de ligamento periodontal entre os fragmentos<\/em>\u00a0e finalmente\u00a0<em>n\u00e3o cicatriza\u00e7\u00e3o com forma\u00e7\u00e3o de tecido de granula\u00e7\u00e3o\u00a0entre os fragmentos<\/em> devido \u00e0 necrose pulpar no fragmento coronal (4). Em 1989, um estudo\u00a0prospectivo\u00a0de 95 dentes permanentes portadores de fratura radicular demonstrou uma rela\u00e7\u00e3o significativa entre a ocorr\u00eancia e tipo de cicatriza\u00e7\u00e3o\u00a0 e o est\u00e1gio de rizog\u00eanese e a extens\u00e3o do deslocamento do fragmento coron\u00e1rio. (23). Al\u00e9m disso, devido aos exames radiogr\u00e1ficos peri\u00f3dicos id\u00eanticos, novas varia\u00e7\u00f5es de cicatriza\u00e7\u00e3o puderam ser descritas, como\u00a0<em>reabsor\u00e7\u00e3o interna\u00a0 de superf\u00edcie e\u00a0reabsor\u00e7\u00e3o interna tipo t\u00fanel<\/em>\u00a0(24).<\/p>\n<p>No in\u00edcio dos anos 70, foi estabelecida uma parceria de pesquisa com o Dr. M. Cvek, do Instituto Odontol\u00f3gico <em>Eastman <\/em>em Estocolmo e foram definidos registros id\u00eanticos para vari\u00e1veis relacionados \u00e0s fraturas radiculares. Em 2005, um estudo prospectivo de 400 dentes portadores de fraturas radiculares permitiu uma an\u00e1lise detalhada <em>do efeito de\u00a0fatores pr\u00e9vios \u00e0 les\u00e3o<\/em>\u00a0(idade, est\u00e1gio de rizog\u00eanese<em>),\u00a0fatores relacionados \u00e0 les\u00e3o<\/em>\u00a0(localiza\u00e7\u00e3o da fratura, gravidade da luxa\u00e7\u00e3o do fragmento coron\u00e1rio) e finalmente\u00a0<em>fatores relacionados ao tratamento\u00a0<\/em>(exatid\u00e3o do reposicionamento, m\u00e9todo e tempo de conten\u00e7\u00e3o e administra\u00e7\u00e3o de antibi\u00f3ticos) (48, 49). Tamb\u00e9m foram analisados os efeitos de v\u00e1rios procedimentos de tratamento endod\u00f4ntico (50). Finalmente, o progn\u00f3stico a longo prazo dos dentes portadores de fraturas radiculares foi publicado em 2008 (59) e 2009 (72).<\/p>\n<h3>Fraturas alveolares<\/h3>\n<p>Em um estudo\u00a0<em>retrospectivo<\/em>\u00a0de 1968 foram analisados 29 fraturas alveolares envolvendo 71 dentes e constatou-se que necrose pulpar e reabsor\u00e7\u00e3o radicular progressiva foram muito frequentes(6). Entretanto, verificou-se posteriormente que os procedimentos de tratamento utilizados \u00e0 \u00e9poca n\u00e3o eram ideais. Em 2009, um estudo prospectivo de 84 fraturas alveolares com 197 dentes envolvidos, demonstrou um risco significativamente menor de complica\u00e7\u00f5es do que qualquer outro estudo relatado anteriormente e especificou um conjunto de preditores significativos para a cicatriza\u00e7\u00e3o e suas complica\u00e7\u00f5es ap\u00f3s fraturas alveolares (62, 63). Al\u00e9m disso, um estudo cl\u00ednico sobre a etiologia e patog\u00eanese, incluindo 340 casos e 800 dentes, foi publicado(61).<\/p>\n<h3>Fraturas de mand\u00edbula com envolvimento do dente<\/h3>\n<p>Em 1969 foi examinada uma s\u00e9rie de 84 fraturas da mand\u00edbula e foi encontrada uma frequ\u00eancia bastante alta de perda \u00f3ssea alveolar nas fraturas, um achado relacionado principalmente a um reposicionamento insuficiente (9).<\/p>\n<h3>Les\u00f5es de dentes dec\u00edduos<\/h3>\n<p>As complica\u00e7\u00f5es de cicatriza\u00e7\u00e3o pulpar e periodontal para os dentes dec\u00edduos luxados, bem como as implica\u00e7\u00f5es para os sucessores em desenvolvimento foram descritos em dois estudos\u00a0<em>retrospectivos<\/em> do in\u00edcio dos anos 70 (10, 11). Em um estudo cl\u00ednico e histol\u00f3gico combinado, foram descritas oito complica\u00e7\u00f5es diferentes envolvendo os sucessores permanentes subjacentes. Estas englobavam <em>manchas opacas do esmalte,\u00a0hipoplasia do esmalte com ou sem hipoplasia circunferencial do esmalte cervical,\u00a0dilacera\u00e7\u00f5es da coroa ou raiz,\u00a0angula\u00e7\u00f5es da raiz,\u00a0interrup\u00e7\u00e3o da forma\u00e7\u00e3o da radicular e\u00a0sequestro de germes dent\u00e1rios infectados.<\/em> (10, 14).<\/p>\n<p>Com base nestes estudos tentou-se desenvolver um tratamento baseado em evid\u00eancias para les\u00f5es dent\u00e1rias dec\u00edduas usando um modelo experimental com macacos. Os efeitos de procedimentos de tratamento como extra\u00e7\u00e3o ou espera de erup\u00e7\u00e3o espont\u00e2nea foram examinados em v\u00e1rios experimentos (85, 86). Al\u00e9m disso, foram analisados os efeitos prejudiciais da inflama\u00e7\u00e3o cr\u00f4nica dos dentes dec\u00edduos causada por uma necrose pulpar n\u00e3o tratada no sucessor permanente(87).<\/p>\n<p>Com base nesses experimentos com animais foi iniciado um estudo\u00a0<em>prospectivo<\/em>\u00a0com 270 crian\u00e7as com dentes dec\u00edduos traumatizados. Todas as crian\u00e7as foram acompanhadas anualmente at\u00e9 os 6 anos de idade, quando foram analisadas todas as complica\u00e7\u00f5es nos dentes prim\u00e1rios afetados e sua rela\u00e7\u00e3o \u00a0com o tipo de luxa\u00e7\u00e3o e o est\u00e1gio de rizog\u00eanese no momento do trauma (39). Aos seis anos de idade foram analisados os problemas de erup\u00e7\u00e3o para os sucessores permanentes. Por fim, os pacientes foram chamados aos 10 anos de idade, quando altera\u00e7\u00f5es posteriores do desenvolvimento do sucessor permanente puderam ser registradas.<\/p>\n<h3>Criando um banco de dados unificado para todos os tipos de traumas<\/h3>\n<p>Desde 1965 o registro sistem\u00e1tico de todos os tipos de trauma foi iniciado sendo que os exames e adocumenta\u00e7\u00e3o das consultas de acompanhamento dos pacientes foram realizados de forma padronizada. Ao todo, existem 40.000 registros de pacientes e cerca de 4.000 foram inclu\u00eddos em estudos de acompanhamento a longo prazo. Esses 4.000 casos fazem agora parte de um extenso banco de dados que abrange todos os tipos de les\u00f5es traum\u00e1ticas dent\u00e1rias. Em 2005 foi iniciado o projeto\u00a0<em>Dental Trauma Guide<\/em>\u00a0a fim de utilizar essa valiosa fonte de informa\u00e7\u00e3o para estimar fatores progn\u00f3sticos de complica\u00e7\u00f5es ap\u00f3s o traumatismo dent\u00e1rio. Os dados permitem calcular estimativas de progn\u00f3stico para os v\u00e1rios tipos de les\u00f5es e fazer compara\u00e7\u00f5es entre os procedimentos de tratamento concorrentes. O objetivo e fun\u00e7\u00e3o do\u00a0<em>Dental Trauma Guide<\/em> ser\u00e1 descrito em uma publica\u00e7\u00e3o futura.<\/p>\n<p>As estimativas de risco foram baseadas em estat\u00edsticas de sobrevida realizadas com pacientes selecionados a partir do banco de dados, identificados como tendo um cen\u00e1rio de trauma compar\u00e1vel.
