{"id":160102,"date":"2020-05-18T13:35:05","date_gmt":"2020-05-18T13:35:05","guid":{"rendered":"https:\/\/dentaltraumaguide.org\/historia\/"},"modified":"2021-01-06T08:19:23","modified_gmt":"2021-01-06T08:19:23","slug":"history","status":"publish","type":"page","link":"https:\/\/dentaltraumaguide.org\/es\/history\/","title":{"rendered":"Historia"},"content":{"rendered":"<h2>Historia de <em>Dental Trauma Guide<\/em><\/h2>\n<p>En 1962 se tom\u00f3 una decisi\u00f3n crucial en el Centro de Trauma del Departamento de Cirug\u00eda Oral y M\u00e1xilofacial del Hospital Universitario de Copenhague: Se llevar\u00eda a cabo la elaboraci\u00f3n de registros de seguimiento peri\u00f3dico de todos los pacientes tratados por lesiones traum\u00e1ticas dentales y maxilofaciales. En aquel momento, dicho departamento ten\u00eda como \u00fanica responsabilidad el tratamiento de las lesiones traum\u00e1ticas dentales en la zona oriental de Dinamarca, cuya poblaci\u00f3n era de casi dos millones de habitantes.<\/p>\n<p>Las pautas de tratamiento que se aplicaban en aquel momento se basaban en el texto de 1960 <i>\u00abThe Classification and Treatment of Traumatic Dental Injuries\u00bb<\/i>, de R.G. Ellis (1), libro compuesto por casos cl\u00ednicos y opiniones de expertos. Este bajo nivel de evidencia era la base para la toma de decisiones en la elecci\u00f3n de tratamiento, debido a la ausencia de material cient\u00edfico de calidad en el campo de la traumatolog\u00eda dental.<\/p>\n<p>Pronto se hizo evidente a trav\u00e9s de las visitas de seguimiento de pacientes de trauma que los reg\u00edmenes basados en dichas fuentes de informaci\u00f3n con frecuencia resultaban en graves complicaciones en la cicatrizaci\u00f3n. Tales complicaciones se pusieron de relieve en una serie de estudios retrospectivos que inclu\u00edan\u00a0<em>luxaciones<\/em>\u00a0(7),\u00a0<em>avulsiones<\/em>\u00a0con el posterior\u00a0<em>reimplante<\/em>\u00a0(2, 3),\u00a0<em>fractura radicular\u00a0<\/em>(4, 5),\u00a0<em>fractura alveolar<\/em>\u00a0(6) y\u00a0<em>fractura mandibular<\/em>\u00a0(6). Las complicaciones en la cicatrizaci\u00f3n que se reconocieron en dichos controles cl\u00ednicos y radiogr\u00e1ficos de seguimiento fueron\u00a0<em>necrosis pulpar<\/em>,\u00a0<em>obliteraci\u00f3n del conducto radicular<\/em>,\u00a0<em>reabsorci\u00f3n radicular progresiva<\/em>,\u00a0<em>p\u00e9rdida de soporte \u00f3seo<\/em>\u00a0y\u00a0<em>p\u00e9rdida dentaria<\/em>.<\/p>\n<p>El intento por establecer una relaci\u00f3n de causa y efecto entre el tipo y la extensi\u00f3n de la lesi\u00f3n, la modalidad de tratamiento y la posterior aparici\u00f3n de complicaciones se vio gravemente obstaculizado por la falta de informaci\u00f3n completa en la historia del paciente, como as\u00ed tambi\u00e9n por la insuficiente documentaci\u00f3n radiogr\u00e1fica inicial sobre la extensi\u00f3n de la lesi\u00f3n. El primer paso para rectificar esta situaci\u00f3n fue establecer un sistema de clasificaci\u00f3n para los traumatismos dentales que abarcara todas las posibles lesiones de tejidos duros dentarios, tejidos periodontales y del hueso alveolar. Se opt\u00f3 por utilizar como modelo el sistema de clasificaci\u00f3n de la OMS y se agregaron algunos traumatismos faltantes (concusi\u00f3n, subluxaci\u00f3n y luxaci\u00f3n lateral) a fin de cubrir todos los escenarios biol\u00f3gicamente distinguibles en cuanto a lesiones dentales (147). Desde entonces, dicho sistema de clasificaci\u00f3n ha gozado de una amplia aceptaci\u00f3n y se ha utilizado en todos nuestros estudios de trauma posteriores.<\/p>\n<p>A fin de mejorar la calidad de la historia de trauma de los pacientes del Hospital Universitario y de incrementar su valor para investigaci\u00f3n, en 1966 se desarroll\u00f3 e implement\u00f3 un formulario est\u00e1ndar para traumatolog\u00eda. Este registro estandarizado de los pacientes era considerablemente exhaustivo y conten\u00eda todos los detalles necesarios sobre el tipo y la extensi\u00f3n de la lesi\u00f3n, las caracter\u00edsticas del paciente (por ejemplo, etapa de desarrollo de la ra\u00edz, edad y sexo), como as\u00ed tambi\u00e9n informaci\u00f3n sobre el tratamiento proporcionado. En total, por cada paciente se recogieron 65 datos sobre el traumatismo. En 1969 de desarroll\u00f3 un\u00a0<em>examen radiogr\u00e1fico est\u00e1ndar<\/em>\u00a0que utilizaba cuatro proyecciones radiogr\u00e1ficas de cada diente traumatizado y en 1971 se agreg\u00f3 el\u00a0<em>registro fotogr\u00e1fico<\/em>\u00a0a fin de incluir una fotograf\u00eda facial de la regi\u00f3n oral para registrar lesiones labiales asociadas, como as\u00ed tambi\u00e9n fotos frontales y axiales de los dientes lesionados (16). Como complemento de los registros iniciales, se realizaron procedimientos de seguimiento a intervalos predeterminados, elegidos en base a la experiencia como aquellos que representaban los momentos \u00f3ptimos para reconocer complicaciones en la cicatrizaci\u00f3n. Ante el fracaso del tratamiento y la extracci\u00f3n dentaria, los dientes eran examinadas en el Departamento de Patolog\u00eda Oral de la Escuela de Odontolog\u00eda, la cual manten\u00eda fuertes lazos con el Hospital Universitario (el principal investigador cumpl\u00eda funciones en ambas instituciones). Para las d\u00e9cadas de los setenta y ochenta se hab\u00eda reunido suficiente informaci\u00f3n estandarizada como para permitir la realizaci\u00f3n de\u00a0<em>estudios cl\u00ednicos \u00a0de trauma prospectivos<\/em>\u00a0y en total se han completado hasta el momento ochenta publicaciones que cubren todas las lesiones traum\u00e1ticas que afectan la dentici\u00f3n temporal y permanente (2-80, 156).<\/p>\n<p>A principios de los a\u00f1os setenta se present\u00f3 una oportunidad \u00fanica para llevar a cabo <i>traumatolog\u00eda dental experimental<\/i> cuando se le permiti\u00f3 al investigador principal realizar estudios en monos utilizados en el Instituto de Sueros de Dinamarca. Los animales se utilizaban como donantes renales para la producci\u00f3n de la vacuna contra la poliomielitis. Los experimentos realizados con dichos primates dieron como resultado 65 estudios de traumatolog\u00eda dental, entre ellos algunos estudios in vitro (81-146).<\/p>\n<p>En la siguiente secci\u00f3n se describir\u00e1 el volumen de informaci\u00f3n cl\u00ednica y experimental disponible para cada tipo de traumatismo. Dicha informaci\u00f3n es esencial, ya que da forma al m\u00e9todo que utiliza\u00a0<em>Dental Trauma Guide<\/em>\u00a0para cada lesi\u00f3n de trauma individual en cuanto al\u00a0<em>diagn\u00f3stico<\/em>\u00a0y la\u00a0<em>elecci\u00f3n de tratamiento<\/em>\u00a0y la\u00a0<em>estimaci\u00f3n del pron\u00f3stico<\/em>.<\/p>\n<h3>Luxaciones<\/h3>\n<p>La concusi\u00f3n, subluxaci\u00f3n, luxaci\u00f3n extrusiva, luxaci\u00f3n lateral y luxaci\u00f3n intrusiva que comprometen la dentici\u00f3n permanente se analizaron en una serie de estudios prospectivos (16, 17, 20, 21, 22, 66-69) y las complicaciones en la cicatrizaci\u00f3n, como la necrosis pulpar, la obliteraci\u00f3n del conducto radicular (22), la reabsorci\u00f3n radicular externa e interna (29), la p\u00e9rdida \u00f3sea marginal transitoria o permanente (16, 17) y la p\u00e9rdida \u00f3sea apical transitoria fueron analizadas en detalle (21-46). La etiolog\u00eda y patog\u00e9nesis de la\u00a0<em>necrosis pulpar<\/em>\u00a0se analiz\u00f3 especialmente en relaci\u00f3n con el tipo de luxaci\u00f3n y la etapa de desarrollo radicular (17). La etiolog\u00eda y patog\u00e9nesis de la\u00a0<em>obliteraci\u00f3n del conducto radicular<\/em> (OCR) constituy\u00f3 otra de las complicaciones en la cicatrizaci\u00f3n estudiadas y se encontr\u00f3 una estrecha relaci\u00f3n entre la ocurrencia de la OCR y el desplazamiento inicial (reducci\u00f3n del flujo sangu\u00edneo apical) y la etapa de desarrollo radicular (22). Los efectos a largo plazo de la obliteraci\u00f3n del conducto radicular (es decir, hasta 20 a\u00f1os) se analizaron en un estudio conjunto con la Dra. A. Robertson de la Escuela de Odontolog\u00eda de Gotemburgo, Suecia, y se estableci\u00f3 la existencia de un riesgo anual levemente aumentado de posterior necrosis pulpar en los dientes con obliteraci\u00f3n del conducto radicular (37).<br \/>\nSe analizaron las complicaciones de la cicatrizaci\u00f3n periodontal y se hall\u00f3 una estrecha relaci\u00f3n entre los eventos de reabsorci\u00f3n radicular y el tipo de luxaci\u00f3n (66-69). Adem\u00e1s, en el caso de las intrusiones, se identific\u00f3 una relaci\u00f3n entre la elecci\u00f3n de tratamiento inicial y la frecuencia de la posterior reabsorci\u00f3n radicular (51-53).<\/p>\n<h3>Avulsiones con posterior reimplante<\/h3>\n<p>El primer estudio cl\u00ednico\u00a0<em>retrospectivo<\/em>\u00a0de 110 dientes reimplantados se public\u00f3 en 1966 y mostr\u00f3 una relaci\u00f3n significativa entre el per\u00edodo de tiempo seco extraoral y las posteriores complicaciones de la cicatrizaci\u00f3n, como ser necrosis pulpar y reabsorci\u00f3n radicular progresiva (2, 3). Un estudio sobre 22 dientes humanos extra\u00eddos y analizados histol\u00f3gicamente mostr\u00f3 que los eventos de reabsorci\u00f3n radicular podr\u00edan ser divididos en reabsorci\u00f3n superficial, reabsorci\u00f3n inflamatoria y reabsorci\u00f3n por reemplazo (2, 3). Dicha terminolog\u00eda luego cambi\u00f3 a\u00a0<em>reabsorci\u00f3n asociada a reparaci\u00f3n<\/em>,\u00a0<em>reabsorci\u00f3n inflamatoria asociada a infecci\u00f3n<\/em>\u00a0y\u00a0<em>reabsorci\u00f3n asociada a anquilosis (reemplazo \u00f3seo)<\/em>.<\/p>\n<p>Un estudio histol\u00f3gico demostr\u00f3 que la reabsorci\u00f3n asociada a infecci\u00f3n se origina en la presencia de bacterias localizadas en el conducto radicular y los t\u00fabulos dentinarios (3) y un estudio cl\u00ednico mostr\u00f3 que el tratamiento de conducto radicular tradicional podr\u00eda prevenir y detener la reabsorci\u00f3n asociada a infecci\u00f3n en alrededor del 10% de los casos (2). Algunos a\u00f1os m\u00e1s tarde se descubri\u00f3 que el hidr\u00f3xido de calcio era efectivo para prevenir y detener la reabsorci\u00f3n asociada a infecci\u00f3n en el 90% de los casos (12).<\/p>\n<p>A principios de los a\u00f1os setenta, se inici\u00f3 una serie de experimentos en primates a fin de estudiar la patog\u00e9nesis de los tipos de reabsorci\u00f3n radicular posterior al reimplante (88-99) y para evaluar los efectos de diversos tratamientos sobre el resultado del reimplante de dientes permanentes. Entre los procedimientos y factores analizados se encuentran los efectos de eliminar el\u00a0<em>co\u00e1gulo<\/em>\u00a0antes del reimplante (93), el efecto negativo causado por\u00a0<em>el tiempo extraoral seco y h\u00famedo<\/em>, la efectividad de diversos\u00a0<em>medios de conservaci\u00f3n<\/em>\u00a0y su\u00a0<em>temperatura<\/em>\u00a0(95, 109, 116, 141), los\u00a0<em>procedimientos de limpieza<\/em>\u00a0de la superficie radicular previo al reimplante (104), la\u00a0<em>precisi\u00f3n del reposicionamiento<\/em>\u00a0(102, 113), los\u00a0<em>m\u00e9todos de ferulizaci\u00f3n<\/em>\u00a0(r\u00edgida o flexible) y el\u00a0<em>tiempo de ferulizaci\u00f3n<\/em>\u00a0(81, 110), la\u00a0<em>sobrecarga oclusal<\/em>\u00a0durante la cicatrizaci\u00f3n (101), y el\u00a0<em>tratamiento endod\u00f3ntico<\/em>\u00a0previo al reimplante (103, 106, 120, 121, 125-129). En aquel momento, se coordin\u00f3 una investigaci\u00f3n colaborativa concerniente al dise\u00f1o de procedimientos experimentales y clasificaciones histol\u00f3gicas de cicatrizaci\u00f3n entre colegas de la Escuela de Odontolog\u00eda de Estocolmo (Drs. L. Hammarstr\u00f6m, L. Andersson, S. Lindskog, L. Bl\u00f6mlof) y el Dr. M. Cvek del Eastman Dental Institute de Estocolmo (117, 118). Adem\u00e1s, dichos investigadores llevaron a cabo una serie de experimentos en relaci\u00f3n con los efectos de los\u00a0<em>antibi\u00f3ticos <\/em>en la cicatrizaci\u00f3n de la pulpa y del ligamento periodontal luego del reimplante (143-146).<\/p>\n<p>En 1994 se public\u00f3 el primer estudio\u00a0<em>prospectivo<\/em>\u00a0de 400 dientes humanos reimplantados con per\u00edodos de seguimiento de hasta 20 a\u00f1os luego del reimplante. Gracias a la informaci\u00f3n detallada sobre la significativa relaci\u00f3n entre la condici\u00f3n de conservaci\u00f3n extra alveolar y el desarrollo posterior de complicaciones en la cicatrizaci\u00f3n pulpar y periodontal, se logr\u00f3 establecer un detallado an\u00e1lisis de la sobrevida de los dientes reimplantados en ciertas condiciones extra alveolares dadas (33-36). En un estudio posterior se describi\u00f3 la influencia de diversos factores como la edad, la ferulizaci\u00f3n y las t\u00e9cnicas endod\u00f3nticas sobre el progreso o detenci\u00f3n de la reabsorci\u00f3n radicular (64, 65).<\/p>\n<h3>Fracturas coronarias con y sin exposici\u00f3n de la pulpa<\/h3>\n<p>Actualmente estamos trabajando en un estudio prospectivo que abarca 853 dientes permanentes e incluye los cinco tipos de fracturas coronarias (infracciones, fracturas del esmalte, fracturas de esmalte y dentina, y fracturas de esmalte, dentina y pulpa) como as\u00ed tambi\u00e9n las combinaciones con los cinco tipos de luxaci\u00f3n anteriormente mencionados (76-80). Para todos los tipos de fracturas y sus combinaciones se encuentran documentadas las tasas de cicatrizaci\u00f3n en relaci\u00f3n con la cicatrizaci\u00f3n de la pulpa y del ligamento periodontal.<\/p>\n<p>En 1985 los doctores Asmussen y Munksgaard de la Escuela de Odontolog\u00eda de Copenhague desarrollaron un nuevo sistema de adhesi\u00f3n (Gluma\u00ae) en cooperaci\u00f3n con la empresa ESPE\u00ae y se decidi\u00f3 utilizar dicho sistema para reponer fragmentos dentarios fracturados (27, 28). En 1991, un estudio multic\u00e9ntrico escandinavo (Gotemburgo, Suecia, Oslo, Noruega y Copenhague, Dinamarca) mostr\u00f3 que con este m\u00e9todo se obten\u00edan algunos resultados prometedores a largo plazo (31) pero con frecuencia los fragmentos adheridos se desprend\u00edan debido a la masticaci\u00f3n o a un nuevo traumatismo, eventos que indicaban la necesidad de mejorar a\u00fan m\u00e1s el procedimiento de adhesi\u00f3n, lo cual implic\u00f3 la realizaci\u00f3n de una serie de nuevos estudios\u00a0<em>in vitro<\/em>\u00a0(132-137).<\/p>\n<h3>Fracturas corono-radiculares<\/h3>\n<p>Las dos estrategias terap\u00e9uticas m\u00e1s utilizadas en el tratamiento de fracturas corono-radiculares no complicadas y complicadas, a saber, restauraci\u00f3n luego de\u00a0<em>gingivectom\u00eda<\/em>\u00a0\u00b1\u00a0<em>osteotom\u00eda<\/em>\u00a0y\u00a0<em>extrusi\u00f3n ortod\u00f3ncica<\/em>\u00a0+\u00a0<em>gingivectom\u00eda<\/em>, se analizaron en un estudio prospectivo a largo plazo. En relaci\u00f3n con la estabilidad del diente restaurado en la arcada dentaria y la salud gingival asociada a \u00e9ste, se encontraron diferencias significativas entre ambos m\u00e9todos, en favor de la extrusi\u00f3n ortod\u00f3ncica en cuanto a la cicatrizaci\u00f3n a largo plazo (70).<\/p>\n<h3>Fracturas radiculares<\/h3>\n<p>En 1967 el primer estudio\u00a0<em>retrospectivo<\/em>\u00a0cl\u00ednico, radiogr\u00e1fico e histol\u00f3gico de 50 dientes permanentes anteriores con fractura radicular revel\u00f3 tres tipos principales de escenarios de cicatrizaci\u00f3n, a saber,\u00a0<em>cicatrizaci\u00f3n con tejido duro entre fragmentos<\/em>,\u00a0cicatrizaci\u00f3n <em>con tejido entre fragmentos derivado del ligamento periodontal<\/em>\u00a0y finalmente,\u00a0ausencia<em> de cicatrizaci\u00f3n con desarrollo de tejido de granulaci\u00f3n entre los fragmentos<\/em>\u00a0debido a la necrosis de la pulpa del fragmento coronario (4). En 1989 se complet\u00f3 un estudio\u00a0<em>prospectivo<\/em>\u00a0de 95 dientes permanentes con fractura radicular y se demostr\u00f3 la significativa relaci\u00f3n entre el resultado de la cicatrizaci\u00f3n, la etapa de desarrollo radicular y la extensi\u00f3n de la dislocaci\u00f3n. (23). Adem\u00e1s, gracias a los ex\u00e1menes radiogr\u00e1ficos peri\u00f3dicos id\u00e9nticos, se pudieron describir nuevas variantes de la cicatrizaci\u00f3n, como ser la\u00a0<em>reabsorci\u00f3n superficial interna<\/em>\u00a0y la<em> reabsorci\u00f3n en tunelizaci\u00f3n interna<\/em>\u00a0(24).<\/p>\n<p>A principios de los a\u00f1os setenta se estableci\u00f3 una colaboraci\u00f3n en investigaci\u00f3n con el Dr. M. Cvek del Eastman Dental Institute de Estocolmo y se organizaron los registros id\u00e9nticos de variables relacionadas con las fracturas radiculares. En 2005, un estudio prospectivo de 400 dientes con fractura radicular permiti\u00f3 el an\u00e1lisis de los\u00a0<em>factores previos a la lesi\u00f3n<\/em>\u00a0(edad, desarrollo radicular), los\u00a0<em>factores de la lesi\u00f3n<\/em>\u00a0(localizaci\u00f3n de la fractura, severidad de la dislocaci\u00f3n del fragmento) y finalmente los\u00a0<em>factores dependientes del tratamiento<\/em>\u00a0(reposicionamiento completo, m\u00e9todo y tiempo de ferulizaci\u00f3n y administraci\u00f3n de antibi\u00f3ticos) (48, 49). Adem\u00e1s, se analizaron los efectos de distintos procedimientos de tratamiento endod\u00f3ntico (50). Finalmente, la evoluci\u00f3n a largo plazo de los dientes con fractura radicular se public\u00f3 en 2008 (59) y 2009 (72).<\/p>\n<h3>Fracturas alveolares<\/h3>\n<p>En un estudio <i>retrospectivo<\/i> realizado en 1968, se analiz\u00f3 la evoluci\u00f3n de 29 fracturas alveolares que involucraban 71 dientes, y se observ\u00f3 que la necrosis pulpar y la reabsorci\u00f3n radicular progresiva eran muy frecuentes (6). Sin embargo, posteriormente se demostr\u00f3 que los tratamientos utilizados en aquel momento eran sub\u00f3ptimos. En 2009 se llev\u00f3 a cabo un estudio prospectivo de 84 fracturas alveolares que involucraban 197 dientes. Dicho estudio mostr\u00f3 un riesgo de complicaciones significativamente menor que cualquier otro estudio previo conocido y especific\u00f3 un conjunto de predictores relevantes para las complicaciones en la cicatrizaci\u00f3n luego de una fractura alveolar (62, 63). Asimismo, se public\u00f3 un estudio cl\u00ednico de la etiolog\u00eda y patog\u00e9nesis de 340 casos que involucraban 800 dientes (61).<\/p>\n<h3>Fracturas mandibulares con compromiso dentario<\/h3>\n<p>En 1969 se examin\u00f3 una serie de 84 fracturas mandibulares y se encontr\u00f3 una frecuencia relativamente alta de p\u00e9rdida de hueso alveolar en las fracturas, fen\u00f3meno relacionado principalmente con un reposicionamiento insuficiente (9).<\/p>\n<h3>Lesiones de los dientes temporales<\/h3>\n<p>En dos <i>estudios retrospectivos<\/i> de principios de los a\u00f1os setenta se describi\u00f3 la evoluci\u00f3n de las complicaciones de la cicatrizaci\u00f3n pulpar y periodontal para dientes temporales luxados como as\u00ed tambi\u00e9n las implicaciones para los sucesores en desarrollo (10, 11). En un estudio cl\u00ednico e histol\u00f3gico combinado se describieron ocho diferentes complicaciones que involucraban a los sucesores permanentes subyacentes. Dichas complicaciones inclu\u00edan <i>opacidad del esmalte<\/i>, <i>hipoplasia del esmalte con o sin hipoplasia circular del esmalte cervical<\/i>,<i> dilaceraci\u00f3n coronaria o radicular<\/i>, <i>angulaci\u00f3n radicular<\/i>, <i>detenci\u00f3n de la formaci\u00f3n radicular<\/i> y <i>secuestro del germen dentario infectado<\/i> (10, 14).<\/p>\n<p>Con base en dichos estudios, se decidi\u00f3 intentar el desarrollo de tratamientos basados en la evidencia para las lesiones de la dentici\u00f3n temporal mediante el uso de modelos en primates. En diversos experimentos se analiz\u00f3 el efecto de tratamientos como la extracci\u00f3n o la espera de la erupci\u00f3n espont\u00e1nea (85, 86). Adem\u00e1s, se analizaron los efectos perjudiciales sobre el sucesor permanente causados por la inflamaci\u00f3n cr\u00f3nica de los dientes temporales debido a una necrosis pulpar no tratada (87).<\/p>\n<p>Con base en dichos experimentos en animales se inici\u00f3 un estudio\u00a0<em>prospectivo<\/em>\u00a0sobre 270 ni\u00f1os que hab\u00edan sufrido lesiones de la dentici\u00f3n temporal. Se hizo un seguimiento con controles anuales de todos los ni\u00f1os hasta la edad de 6 a\u00f1os, y luego se analizaron todas las complicaciones de la cicatrizaci\u00f3n que afectaron a los dientes temporales lesionados y su relaci\u00f3n con el tipo de luxaci\u00f3n y la etapa de desarrollo radicular del diente afectado al momento de la lesi\u00f3n (39). A la edad de seis a\u00f1os, se analizaron los problemas de la erupci\u00f3n de la dentici\u00f3n sucesora correspondiente. Finalmente, se contact\u00f3 nuevamente a los pacientes a la edad de 10 a\u00f1os para registrar cambios posteriores en el desarrollo del diente sucesor permanente.<\/p>\n<h3>Confecci\u00f3n de una base de datos unificada para todos los tipos de traumatismos<\/h3>\n<p>Desde el inicio del registro sistem\u00e1tico de los diversos tipos de traumatismos en 1965, se ha examinado y realizado seguimiento a los pacientes mediante la utilizaci\u00f3n de un registro de trauma estandarizado. En total existen 40.000 registros de pacientes y aproximadamente 4000 se han incluido en estudios de seguimiento a largo plazo. Estos 4000 casos son ahora parte de una extensa base de datos que cubre todos los tipos de traumatismos dentales. En 2005 se inici\u00f3 el proyecto de\u00a0<em>Dental Trauma Guide<\/em>\u00a0a fin de utilizar esta valiosa fuente de informaci\u00f3n para la estimaci\u00f3n del pron\u00f3stico de las complicaciones en la cicatrizaci\u00f3n luego del traumatismo dental. Dicha informaci\u00f3n hace posible el c\u00e1lculo de estimaciones pron\u00f3sticas para los diversos tipos de lesiones y la comparaci\u00f3n entre tratamientos contrapuestos. El objetivo y la funci\u00f3n de\u00a0<em>Dental Trauma Guide<\/em>\u00a0se describir\u00e1n en una futura publicaci\u00f3n.<\/p>\n<p>La estimaci\u00f3n del riesgo se basa en las estad\u00edsticas de sobrevida obtenidas a partir de una subselecci\u00f3n de pacientes identificados dentro la base de datos como aquellos que presentaban un escenario de trauma comparable.
